You may get a W-7 or W-8 form by going to the IRS website and searching under forms for the W-7 or W-8 form. You must present the filled out form, original passport, and one copy of each of the following pages: the biographical information page, renewal page (if applicable), visa page (if applicable). Copies should be on one side only of a sheet of paper, and there should be room for the notary seal.
