Hours

The American Citizen Services Unit provides passport services from 8:00 a.m. to 10:30 a.m. Monday through Friday, except holidays and the monthly staff training day. In order to provide a more efficient service we recommend to arrive at the American Citizen Services Unit at 8:00 am with all the forms filled out and the required documents.

NOTE: As of April 8, 2002, except in emergencies, all US passports are printed at facilities in the United States. This change increased processing time by approximately two weeks. Please take time into consideration when applying for or renewing passports at the Embassy, and plan your travel accordingly. You may pick up your new passport in person at the Embassy or arrange to have it delivered by private delivery service (Domesa) for a fee of BsF. 36.00 (delivery will take approximately one week).

In cases where an American citizen needs a passport for immediate travel because of a true emergency, the Embassy may issue a locally-produced passport with a validity not to exceed one year. Examples of such emergencies might include illness or death of a close family member, or (for tourists only) imminent scheduled return travel to the U.S. after a passport has been lost or stolen (NOTE: the “imminent scheduled return travel” exception does NOT apply to U.S. citizens residing in Venezuela.) The emergency passport will be available for pick up 24 hours after the day of the passport application.

Citizenship Claim

ADULT APPLICANT (AGE 16 AND OVER):

There are two different forms to apply for a new passport. Please read carefully the following requirements to determine which form you have to submit:

You must use form DS-82 if you:

(1) Can submit your current passport.
(2) Your current passport was issued to you when you were 16 years of age or older.
(3) Your current passport was issued less than 15 years ago.
(4) Your name has not changed.

NOTE: if you got married or divorced and you want to change your name in your new passport, you may do so by providing the marriage certificate or divorce decree.

We encourage adult applicants to renew their passports personally at the Embassy. You may NOT send your application materials by mail. However, if you choose, you may have someone else complete the application process for you at the Embassy. The fee for an adult passport renewal is $75.00. The fee can be paid in cash, either Dollars or Bolívares, or with a credit card issued in the US. Checks and debit cards are not accepted.

The consular officer may insist that you present additional documentation after reviewing your application, but at a minimum, you must bring the following documentation:

  • Your most recent US passport.
  • Up to date photo ID (if you are recognizable) issued by a government entity, such as a valid Venezuelan cedula, a Venezuelan or other foreign passport, or a driver’s license issued in the United States (mutilated, altered, or damaged identifications are NOT acceptable as proof of identity). Your Social Security Card does NOT prove your identity.
  • Form DS-82 filled out using capital letters in BLACK INK and SIGNED.
  • You must submit two color passport photos, size 2 inches x 2 inches (5 cm x 5 cm), taken within the last six months. The photos must have a white background. The photos cannot have any visible grain or dots from the printing process. The photos must have a glossy finish.
  • $75.00 or its equivalent in Bolívares.

NOTE: if you choose, you may have someone else complete the application process for you at the Embassy as long as your current passport is still valid. If you send it with a third party, make sure you include a letter authorizing the person to complete the application process for you at the Embassy. You must also indicate in the letter if you wish the third party to arrange delivery of the new passport by Domesa or if the new passport will be picked up in person at the embassy. The Domesa fee is not included in the passport fee. Please note that the consular officer can require your personal appearance if he or she has any doubts about your eligibility for a new passport.

IMPORTANT: 26 U.S.C. 6039E (Internal Revenue Code) requires a passport applicant to provide his or her Name and Social Security Number. Any applicant who fails to provide the required information is subject to a $500 penalty enforced by IRS.

If you don’t meet the above mentioned requirements and you are an adult Applicant (age 16 and over) you must use form DS-11 to renew your passport.

You must appear in person before a consular officer at the Embassy to apply for your passport during walk-in hours. The passport fee is $100.00. The fee can be paid in cash, either Dollars or Bolívares, or with a credit card issued in the US. Checks and debit cards are not accepted.

The consular officer may require that you present additional documentation after reviewing your application, but at a minimum, you must bring the original and one copy of the following documents:

  • Your most recent US passport.
  • Up to date photo ID (if you are recognizable) issued by a government entity, such as a valid Venezuelan cedula, a Venezuelan or other foreign passport, or a driver’s license issued in the United States (mutilated, altered, or damaged identifications are NOT acceptable as proof of identity). Your Social Security Card does NOT prove your identity. Applicants from 16 to 18 years of age who do not have photo ID must be accompanied by at least one of their parents who must present their own valid photo ID and the birth certificate of the applicant.
  • If the applicant has dual nationality, bring the valid passport from the other country of citizenship.
  • In case you were born in the United States, you must submit the passport you used to exit U.S. if it is available. If your parents are not U.S. citizens, you MUST also bring the foreign passport that they used to enter the U.S. before giving birth to you.
  • Form DS-11 filled out using capital letters in BLACK INK and NOT SIGNED
  • You must submit two color passport photos, size 2 inches x 2 inches

PASSPORT RENEWALS FOR MINORS (15 YEARS OF AGE AND YOUNGER):

IMPORTANT NOTE: APPLICANTS YOUNGER THAN 16 MUST BE ACCOMPANNIED BY BOTH PARENTS WHO MUST EACH PRESENT A VALID PHOTO ID. ISSUED BY A GOVERNMENT ENTITY, SUCH AS A U.S. PASSPORT OR OTHER FOREIGN PASSPORT OR U.S. STATE DRIVERS LICENSE, A VENEZUELAN CEDULA OR PASSPORT. IF ONE PARENT CANNOT COME, AND THE APPLYING PARENT DOES NOT HAVE AN OFFICIAL DOCUMENT SHOWING THAT HE OR SHE HAS SOLE CUSTODY OF THE CHILD, A NOTARIZED STATEMENT OF CONSENT AND A COPY OF HIS/HER ID IS REQUIRED FROM THE NON-APPEARING PARENT. THIS STATEMENT MUST INDICATE THE PARENT'S UNEQUIVOCAL AND UNCONDITIONAL CONSENT TO PASSPORT ISSUANCE. STATEMENTS NOTARIZED BY FOREIGN OFFICIALS WILL GENERALLY BE ACCEPTED BUT THE STATEMENT MUST BE IN ENGLISH. APPLYING PARENTS MAY USE THE FORM SUPPLIED BY THE DEPARTMENT OF STATE ( DS-3053) OR AN AFFIDAVIT OF THEIR OWN.

Applicants for passport renewals age 16 or younger must appear in person before a consular officer at the Embassy.

The proper passport application form for a passport renewal for an applicant age 15 or under is a DS-11. The passport fee is $85.00 and it can be paid in cash, either Dollars or Bolívares, or with a credit card issued in the US. Checks and debit cards are not accepted.

The consular officer may insist that you present additional documentation after reviewing the child’s passport application, but at a minimum, you must bring the original and one copy of the following documents:

  • Child’s most recent US passport.
  • The original or a certified copy of the child’s U.S. Birth certificate or a Consular Report of Birth Abroad of a Citizen of the United States of America (if applicable). Mutilated, altered, or damaged birth certificates or CRBAs will NOT be accepted. These documents must show the name of the parents (only for applicants age 13 or younger).
  • Proof of the child's U.S. citizenship, such as a previous U.S. passport (mutilated, altered, or damaged passports are NOT acceptable as proof of U.S. citizenship), a Naturalization Certificate or a Certificate of Citizenship.
  • If the applicant has dual nationality, bring the valid passport from the other country of citizenship.
  • In case you were born in the United States, you must submit the passport you used to exit U.S. if it is available. If your parents are not U.S. citizens, you MUST also bring the foreign passport that they used to enter the U.S. before giving birth to you.
  • A sequence of 10 to 15 photos covering the period from the time the last passport was issued to the present date.
  • Applicants age 16 and over who do not have photo ID must be accompanied by at least one of their parents who must present their own valid photo ID and the birth certificate of the applicant.

  • Also your Venezuelan birth certificate issued by the Venezuelan Consulate in the United States. Mutilated, altered, or damaged birth certificates will NOT be accepted.
  • Consular Report of Birth Abroad of a Citizen of the United States of America (if applicable). Mutilated, altered, or damaged CRBAs will NOT be accepted.
  • Proof of U.S. citizenship, such as a previous U.S. passport (mutilated, altered, or damaged passports are not acceptable as proof of U.S. citizenship), a U.S. Birth certificate, Consular Report of Birth Abroad of a Citizen of the United States of America (Mutilated, altered, or damaged birth certificates or CRBAs will NOT be accepted), Naturalization Certificate or a Certificate of Citizenship.

TO ADD PAGES TO A VALID U.S. PASSPORT

Applications to add visa pages to U.S. passports are handled by the American Citizen Services Unit during our regular passport services hours. Only one set of extra pages (24 pages are in each set) can be added to your passport at a time, but if needed, and if your passport remains in good condition, you may have up to three sets of extra pages inserted into your passport. Once this limit is reached, you will need to apply for passport renewal. You may submit your passport and application personally or through a third party. This process normally takes 24-48 hours. There is no fee for this service.

You must submit the signed form DS-4085 (Passport Amendment/Validation Application) along with your valid passport.

IMPORTANT NOTE: PERSONAL APPEARANCE OF THE MINOR (UNDER THE AGE OF 16) IS NOT REQUIRED FOR THE PURPOSE OF ADDING ADDITIONAL VISA PAGES TO HIS/HER PASSPORT. HOWEVER THE MINOR’S PARENT’S MUST COMPLY WITH THE TWO-PARENT CONSENT REQUIREMENT BY SUBMITTING A NOTARIZED PARENTAL CONSENT FORM (S) SIGNED BY EACH PARENT AND PROVIDING THE MINOR’S ORIGINAL BIRTH CERTIFICATE AND PARENTS IDENTITY DOCUMENTS. NAME CHANGES OR CORRECTIONS ON THE DATA PAGE ON A U.S. PASSPORT

Requests for name changes or corrections on the data page on U.S. passports may be submitted to the American Citizen Services Unit during regular passport services hours from 8:00 a.m. to 10:30 a.m. Monday through Friday.

If you have changed your name, you may apply to have this change reflected in your passport. Please note that as of September 26, 2005, U.S. passports will no longer be amended to change items appearing on the data page; you will therefore apply for a new passport in your new name. If your passport was issued more than 12 months ago and you are 16 years of age or older, you must follow the instructions for Adult Renewal age 16 and over. If your passport was issued more than 12 months ago and you are under 16 years of age you must follow the instructions for Minor Passport Renewal Ages 15 or younger.

If your passport was issued within the last 12 months you must submit:

• Your current U.S. Passport.
• Form DS-5504 filled out using capital letters in BLACK INK and NOT SIGNED. The instructions on this form are written for individuals applying for a passport within the United States. As applications are processed differently outside the United States, you should ignore instruction pages one and two of this form.
• Documentary evidence to support the change. In the case of a name change you may submit a certified court order, marriage certificate or divorce decree. To correct printing errors on the data page you must submit at least TWO (2) identifying documents such as birth certificate, previous passports or up to date photo ID (if you are recognizable) issued by a government entity, such as a valid Venezuelan cedula, a Venezuelan or other foreign passport, or a driver’s license issued in the United States (mutilated, altered, or damaged identifications are NOT acceptable as proof of identity). Your Social Security Card does NOT prove your identity.
• You must submit two color passport photos, size 2 inches x 2 inches
(5 cm x 5 cm), taken within the last six months. The photos must have a white background. The photos cannot have any visible grain or dots from the printing process. The photos must have a glossy finish.

There is no fee for this service. If you are 16 years of age or older, you may have someone else complete the application process for you at the Embassy. If you send it with a third party, make sure you include a letter authorizing the person to complete the application process for you. You must also indicate in the letter if you wish the third party to arrange delivery of the new passport by Domesa or if the new passport will be picked up in person at the embassy. The Domesa fee is not included in the passport fee. Please note that the consular officer can require your personal appearance if he or she has any doubts about your eligibility for a new passport.

IMPORTANT NOTE: APPLICANTS YOUNGER THAN 16 MUST BE ACCOMPANNIED BY BOTH PARENTS WHO MUST EACH PRESENT A VALID PHOTO ID. ISSUED BY A GOVERNMENT ENTITY, SUCH AS A U.S. PASSPORT OR OTHER FOREIGN PASSPORT OR U.S. STATE DRIVERS LICENSE, A VENEZUELAN CEDULA OR PASSPORT. IF ONE PARENT CANNOT COME, AND THE APPLYING PARENT DOES NOT HAVE AN OFFICIAL DOCUMENT SHOWING THAT HE OR SHE HAS SOLE CUSTODY OF THE CHILD, A NOTARIZED STATEMENT OF CONSENT AND A COPY OF HIS/HER ID IS REQUIRED FROM THE NON-APPEARING PARENT. THIS STATEMENT MUST INDICATE THE PARENT'S UNEQUIVOCAL AND UNCONDITIONAL CONSENT TO PASSPORT ISSUANCE. STATEMENTS NOTARIZED BY FOREIGN OFFICIALS WILL GENERALLY BE ACCEPTED BUT THE STATEMENT MUST BE IN ENGLISH. APPLYING PARENTS MAY USE THE FORM SUPPLIED BY THE DEPARTMENT OF STATE (DS-3053) OR AN AFFIDAVIT OF THEIR OWN.

CITIZENSHIP CLAIM

For applicants who have lost their U.S. citizenship and want to regain it, all the documentation pertaining the loss (including the Certificate of Loss of Nationality (CLN) approved by the Department of State) must be provided along with old U.S passports (if available) and a valid passport from other country of citizenship. You may submit your request in the American Citizen Services Unit or send it to the Department of State directly.

The applicant also must submit a written request to review the loss/renunciation of his/her U.S. citizenship addressed to:

Please make sure to include in your letter:

  • Full name (current name as well as any other name you may have used in the past).
  • Place and date of birth.
  • Address and daytime phone numbers where you can be reached.
  • Reason to request a review of the loss/renunciation of your citizenship.
  • Explain the reasons/circumstances of the loss of citizenship.

This letter MUST be in English.

This process may take 4 to 6 months.

The fee for the Passport File search is $60.00 (if making the request through the American Citizen Services Unit). This fee can be paid in cash, either Dollars or Bolívares, or with a credit card issued in the US. Checks and debit cards are not accepted.

An additional fee of $100.00 (Passport fee) is to be paid if the citizenship is restored by the Department of State. You must follow the instructions on applying for a passport for the first time.
