To Report a Lost or Stolen U.S. Passport
Please immediately report the loss of a valid United States Passport by completing a Statement Regarding a Lost or Stolen Passport (PDF 114KB) and submitting it by fax to (0212) 907-8199. If you wish, you may scan a completed form and email it to acsvenezuela@state.gov.

We recommend you also report the loss to Venezuelan law enforcement in the area in which the incident occurred.
Prompt action protects you from any potential misuse of the passport.
To Replace a Lost or Stolen U.S. Passport
You must appear in person at the Embassy or the Consular Agency.
If you need a passport for immediate travel due to a true emergency, and you can submit evidence of such emergency travel plans, you do not require to set up an appointment. If you do not have an immediate travel plan; you have to set up an appointment. Be advice that an individual appointment should be made for each family member seeking a service related to passports
You should submit your application for a replacement passport as soon as possible after the loss or theft is discovered.
The passport fee is $135.00 for applicants 16 and over and $105.00 for applicants under 16. For additional information please see Payment Options.
The consular officer may require that you present additional documentation after reviewing your application, but at a minimum, you must bring the following documents:
- A Statement Regarding Lost or Stolen Passport (PDF 114KB), if you have not already submitted one by fax.
- An Application for a U.S. Passport
- Both documents should be completed in capital letters in black or blue ink. Do not sign them until asked to do so by a consular officer.
- If your lost passport was still valid, we should have a record of it that will serve as proof of your citizenship and identity. However, if there are questions regarding your citizenship or identity the consular officer may request additional evidence of your eligibility for a U.S. passport, such as:
- Proof of U.S. citizenship, such as a previous U.S. passport, a U.S. Birth certificate, a Consular Report of Birth Abroad of a Citizen of the United States of America, Naturalization Certificate or a Certificate of Citizenship. Note: Mutilated, altered or damaged documents will not be accepted as proof of U.S. citizenship.
- A currently valid photo ID issued by a government entity in which you are recognizable, such as a valid Venezuelan cedula, a Venezuelan or other foreign passport, or a driver’s license issued in the United States. Note: Mutilated, altered or damaged identification will not be accepted as proof of identity. Your Social Security Card does not prove your identity,
- Please refer to the special requirements for children under 16 if replacing a lost/stolen passport for a minor.
- Two identical color passport photos that exactly meet the specifications on the passport application.
Important: 26 U.S.C. 6039E (Internal Revenue Code) requires a passport applicant to provide his or her name and Social Security Number. Any applicant who fails to provide the required information is subject to a $500 penalty enforced by IRS.
