Additional requirements for Children

To submit an application for a child under age 16 BOTH parents or the child'slegal guardian(s) must appear and present all of the following:

  • Evidence of the child's U.S. citizenship,
  • Evidence of the child's relationship to parents/guardian(s), AND
  • Parental identification.

If only one parent appears you must also submit one of the following

  • Second parent's notarized written statement in English consenting to passport issuance for the child, applying parents must use the form supplied by the Department of State (DS-3053).
  • These form can only be used in case that the non-present parent is not present Venezuela at the moment of the application)
    Primary evidence of sole authority to apply (e.g. court document stating the appearing parent’s SOLE custody of the child), OR
  • A written statement (made under penalty of perjury) explaining the second parent's unavailability.

Each minor child applying for a passport must appear in person.

In order to apply for a U.S. passport for the first time; you have to set up an appointment. Be advice that an individual appointment should be made for each family member seeking a service related to passports

Applicants must appear in person before a consular officer at the Embassy.

The passport fee is $135.00 for applicants 16 and over and $105.00 for applicants under 16 or the equivalent in local currency. For additional information please see Payment Options.

The consular officer may require that you present additional documentation after reviewing your application, but at a minimum, please bring the original and one copy of the following documents:

  • Form Application for a U.S. Passport DS-11 filled out using capital letters in black or blue ink. Do not sign it.
  • A currently valid, government-issued photo ID in which you are recognizable. Examples would be a Venezuelan Identity Card, non-U.S. passport, or a U.S. driver’s license. Temporary or damaged documents are not acceptable.
  • Proof of U.S. citizenship, such as a U.S. Birth certificate, a Consular Report of Birth Abroad of a Citizen of the United States of America, a Naturalization Certificate or a Certificate of Citizenship. Mutilated or damaged documents are not acceptable.
    • If you are claiming citizenship based on the U.S. citizenship of one of your parents, please provide as much evidence as possible regarding your parent’s U.S. citizenship and physical presence in the United States prior to your birth. If living, your U.S. citizen parent should fill out an Affidavit of Parentage and Parentage (PDF 58.5KB). If your U.S. citizen parent is deceased, you should complete an Affidavit of Physical Presence of a Deceased U.S. Citizen Parent (PDF 169KB). You will also need to provide evidence of your biological relationship. We strongly encourage you to contact our office by e-mail at acsvenezuela@state.gov prior to your visit so that we may discuss documentation needs with you.
    • If you were born in the United States please submit your Venezuelan birth certificate issued by a Venezuelan Consulate in the United States and also the passport you used to exit the United States. If your parents are not U.S. citizens, please bring the foreign passports that they used to enter the U.S. before you were born.
  • All previous IDs (expired or not) you have had in your life, e.g. driver’s licenses, old passports, school IDs, and all other documents that show your name, photo, date and place of birth.
  • Two identical color passport photos that meet the specifications on the application

Important Note: Applicants younger than 16 must be accompanied by both parents who must each present a valid photo ID issued by a government entity, such as a U.S. passport or other foreign passport or U.S. state drivers’ license, a Venezuelan cédula or passport. If one parent cannot come and the applying parent does not have an official document showing that he or she has sole custody of the child, a notarized statement of consent and a copy of his/her ID is required from the non-appearing parent. This statement must indicate the parent's unequivocal and unconditional consent to passport issuance. Statements notarized by foreign officials will generally be accepted but the statement must be in English. Applying parentsmust use the form supplied by the Department of State (ds-3053). (This form can only be used in case that the non-present parent is not in Venezuela at the moment of the application)

 

Important: 26 U.S.C. 6039E (Internal Revenue Code) requires a passport applicant to provide his or her Name and Social Security Number. Any applicant who fails to provide the required information is subject to a $500 penalty enforced by IRS.
