Replacing A Lost, Stolen Or Damaged Ss Card And Name Changes
 

To apply for a replacement card and to correct or change the name shown on the card please complete an Application for a Social Security Card (PDF 163KB) and provide the following documents (Please make sure to bring original documents accompanied by one copy of each):

  • Birth Certificate (if born outside of U.S., you must submit the local birth certificate issued by the foreign authorities)
  • U.S. passport (minimum validity of 6 months)
  • Documents to support the change (for name change cases)

Note: When filling out the Application for a Social Security Card (PDF 163KB) please do not forget to include the COUNTRY in item #2 (Mailing Address). Also enter the four-digits for year in dates (e.g., 2007) and when filling in Zeros, please enter them as Ø, and not just 0.

You may submit the application at the American Citizen Services Unit, Wednesdays from 1:00 p.m. to 3:00 p.m. Please allow at least 6 months to process your application.

Important Note: If you wish to obtain a social security number as soon as possible, you may apply directly at any Social Security Agency in the U.S. For additional information please see the Social Security Administration’s website.
